Outstanding Debt List

If your Program has an ODL matter, the ODL intake Form should be downloaded and completed by the CHC Program’s appointed representative or Board member.  The Intake form should be completed with the required information to support the ODL claim for the program. Programs should pay close attention to the requirements as outlined in the USA Hockey Annual Guide regarding the Notice Requirements and the 30 Day Discussion Period.   Failure to adhere to these provisions will extend the time that it takes to post a player to the ODL Listing.
