NEW ONLINE GAME REPORT SYSTEM 

INSTRUCTIONS TO SUBMIT ALL GAME MISCONDUCTS AND MATCH PENALTIES
 
1. Go to the following website:
            www.chchockey.org
 
2. Put your mouse pointer over the officials tab at the top of the page.
 
3. Move your mouse down to the correct referee group and click on your organization: (Example below)-There are also instructions on this list.
 
 
 
4. You will be prompted for a password.  Enter your association's password.  If you need your association's password, please contact their appropriate rep.
 
5. This will put you into the online form. Please fill out as much information as possible. All fields with an “*” are required fields. 
 
6. Click on <CONTINUE> at the bottom of the page.
 
REMEMBER: You need to get all information off the scoresheet before you leave the rink. Game reports will be automatically forwarded to the commissioners and Supervisors.
 
This is NOT a substitute for a phone call for all match penalties. You are still required to call Randy Zaritsky, Connecticut Supervisor of Officials with all Match penalties-No Exceptions.
